1. New agent is required to furnish the Company with bank account number within 2 months from the date of joining, failing which the entitled monthly commission shall be forfeited automatically by the Company.
2. Uploading the front and back of your IC is optional. A bank account passbook/statement may be supplied later with your bank details.
3. Only application form duly accepted by City Centre is valid. The Company reserves the sole right either to approve or reject an application.
4. Application fee is not refundable whether the application is rejected or it is withdrawn by an agent.
5. Agent shall endeavour not to disclose customer’s information but to keep the same in the highest confidentiality at all times. In the event of legal action resulted in the prosecution against the Company for offenses committed by agent in violation of any provisions under Personal Data Protection Act 2010, all costs, fees, charges and expenses incurred incidental thereto shall be borne by agent absolutely.

I declare that the Company has made available to me, the Company’s Personal Data Protection Notice (“Notice”) as required under the Personal Data Protection Act 2010, and having read and inspected the same, hereby agree that I consent to the Company processing my personal data in accordance with the Notice, and further confirm that I have the authority to provide the personal data of any third parties for processing by the Company in accordance with the Notice.

I hereby certify that all statements made by me in this application and all other information or documents which I have provided are true, accurate and complete to the best of my knowledge. I declare that I have read, understood and agreed to be bound by the Company’s Agency Terms and Conditions, including but not limited to the Company’s Code of Ethics, Procedures and Policies and Rules and Regulations on Agent and Agency (collectively, “Agency Rules & Regulations”), which are made available by the Company and through any policy links provided in this form. I further expressly acknowledge and agree that: (i) the Agency Rules & Regulations form an integral part of my contract with the Company and govern my duties and conduct as an agent; (ii) it is my responsibility to keep myself updated and comply with any changes or updates to the Agency Rules & Regulations; and (iii) any violation or non-compliance may result in disciplinary action by the Company, including warning, demotion, commission deduction, suspension, termination of appointment and/or proceedings as deemed appropriate. I understand and accept that the Company’s decision on any disciplinary action shall be final and binding.
